Job Description

Are you an experienced RN Case Manager looking for a new opportunity with a prestigious healthcare company? Do you want the chance to advance your career by joining a rapidly growing company? If you answered “yes" to any of these questions – this is the position for you!

Job Duties:

  • Responsible for quality and continuous improvement within the job scope.
  • Responsible for all actions/responsibilities as described in company controlled documentation for this position.
  • Contributes to and supports the corporation's quality initiatives by planning, communicating and encouraging team and individual contributions toward the corporation's quality improvement efforts.
  • Responsible for comprehensive patient assessments through health care records, physician and patient interviews.
  • Utilizes motivational interviewing techniques to increase patient engagement and self management breaking down barriers and increasing adherence to the medical regime.
  • Develops, implements, and evaluates comprehensive patient care plans.
  • Responsible for collaboration with healthcare team to provide feedback, updates and recommendations regarding health status and plan of care for patient.
  • Research and recommend specialty services as needed while utilizing contracted vendors or community resources as appropriate.
  • Identify transportation resources and arrange appropriate transportation when needed.
  • Prepares documentation and participates in case review activities. Participates in providing current health status and contributes to interventions and plan of care to reduce re-admission risk and movement towards positive health status.
  • Establish rapport with patient to develop and strengthen trust with the healthcare team.
  • Identify variances and barriers to medical treatment plan and communicates to the PCP/care team. Works collaboratively to provide interventions and solutions.
  • Brings closure to the care and/or episode of illness by discontinuing case management when the highest level of function or best possible outcome has been obtained or the needs of the patient change.
  • Arranges direct admits and facility transfers under the direction of a Provider.
  • Coordinates Home Health and Hospice referrals.
  • Arranges hospital follow-ups within established timeliness.
  • Obtains Hospital/SNF/Specialist and other medical records when needed.
  • Submits admissions and discharges e-mail notifications.
  • Telephonic and face to face screening/education with patients on episodic, high-touch, and low- risk care management.
  • Documents all care coordination communications and interventions.


Requirements:

  • RN license in the state of NV
  • Must have at least 3 years of clinical experience
  • Must have strong computer skills

Hours for this Position:

  • Monday – Friday (8:00a – 5:00p)
