Registered Nurse- Home Care Clinical Lead

Position: RN Clinical Lead- Home Care (Registered Nurse Required) 

Shift: Salaried, Monday-Friday 8am-5pm

On-Call: As Business Needs Require

Experience Needed: Critical Care, Vent, Trach, G-Tube, Geriatrics (Pediatric Experience a Plus) (Home Care or Home Health Experience Preferred) 

Location: Downtown Chicago (Must be willing to commute daily to downtown Chicago) 

Pay: Experience Based $85,000 to $90,000

The ideal candidate combines strong critical care clinical judgment with the flexibility and independence needed to manage a mobile, client-facing schedule. 

This is an exciting opportunity for an experienced home care nurse who understands that exceptional care happens when clinical expertise, strong leadership, and genuine relationships come together.

We are looking for an RN Clinical Lead who knows home care — someone who is comfortable managing complex patients, supporting caregivers in the field, communicating with families and healthcare professionals, and maintaining the highest standards of clinical quality and safety. BrightStar Care of Chicago and La Grange is Joint Commission Accredited. 

Key Responsibilities

• Conduct comprehensive new patient (start of care) assessments in clients' homes


• Perform scheduled and as-needed re-assessments to evaluate changes in patient condition and care needs


• Manage an active caseload, coordinating care plans, updating documentation, and ensuring continuity of care


• Serve as a clinical point of contact for field nurses, answering questions and providing guidance on complex cases


• Communicate with physicians, families, and interdisciplinary team members regarding patient status and care plan changes


• Ensure care plans and documentation meet regulatory and Joint Commission compliance standards


• Identify and escalate clinical concerns or changes in condition appropriately


• Participate in office-based team meetings, case conferences, and administrative work (~1 day/week equivalent)

Qualifications

Required:

• Active, unrestricted RN license in the state of Illinois (Registered Nurse)


• Excellent Communication and Organization Skills


• Minimum 1 year of Critical care clinical experience (Vents, Trachs, G-Tubes, Infusion) 


• Experience with Geriatrics 


• Experience with Pediatrics a plus


• Reliable personal vehicle, valid driver's license, and current auto insurance


• Comfortable with extensive daily driving across the downtown Chicago area and surrounding communities


• Negative TB Test and Physical


• Current CPR Certification


• Strong assessment, critical thinking, and clinical decision-making skills


• Excellent communication skills — comfortable being a clinical resource for peers


• Proficiency with electronic documentation systems

Preferred:

• Minimum 1 year of ICU experience


• Home care, hospice, or case management experience
