Job Title: Registered Nurse – Hospice Home Health

Location: Lawrence, MA

Rate: Up to $2,200 Gross Weekly + benefits available from day 1

Shift: Days, 8:00 AM – 5:00 PM, Monday–Friday

Contract: 13 weeks

Hours: 40 hours/week

Requirements:

• Minimum 1 year of recent hospice experience
• Recent hospice experience required; first-time travelers not accepted
• Active, unencumbered Massachusetts RN license
• Reliable transportation for home visits
• Current driver’s license
• Ability to travel between patient homes

Certifications:

• AHA BLS

Responsibilities:

• Provide skilled hospice nursing care in patients’ homes according to physician orders and care plans
• Assess patients and provide appropriate nursing interventions
• Complete hospice visits, including Start of Care and follow-up visits
• Monitor patient conditions and communicate changes to the appropriate care team
• Provide education and support to patients and their families
• Complete accurate and timely clinical documentation
• Collaborate with physicians and the interdisciplinary care team
