The Clinical Nurse IV (CNIV), is an expert clinician who demonstrates a high level of knowledge and expertise in a specialty as well as general nursing. The CN IV consistently demonstrates excellence in all areas of nursing practice including nursing care delivery, staff education, research, and collaboration. Evaluates and works to improve the process of patient care delivery within the unit and division. The CNIV manages complex, unpredictable patient care situations and is a nursing shift leader. Serves as a clinical and operational resource to the nursing staff, the unit, the organization, and the community to promote the advancement of nursing. The CN IV will assume primary responsibility for the mentoring and coaching of a designated group of employees in one of the three professional advancement pathways (Clinical Quality, Education, Leadership) that most closely aligns with their skillset. The CNIV serves as the expert mentor on the unit with the goal of increasing nurse engagement, professional development of their staff, and safe quality care with improved outcomes.


The scope of responsibility, duties and accountability at the facility department and position levels including knowledge and practice of the clinical/technical skills necessary to the position consistent with the Nurse Practice Act. Nursing job duties are to meet the Nurse Practice Act (state law that protects patients by promoting safe, competent nursing practices), Title 22 (Health and Safety codes), The Joint Commission (develops performance standards and accredits programs for hospitals and other healthcare-related organizations) and the Plan for Provision of Nursing Care and the Plan for Provision of Care to Patients, and other regulatory requirements.


Responsibilities

• Serves as a clinical and operational resource for all nursing activities in the department. When in charge, oversees and authorizes incidental overtime; Provides assistance to co-workers, new staff, temporary staff and students.
• Delegates and assigns tasks to licensed and unlicensed health care workers within the scope of their job responsibilities
• Coaches and supports nursing staff in the application of advanced clinical skills and critical thinking to support patient care activities
• Evaluates, facilitates and promotes patient flow, coordinates discharge planning and team rounding as applicable.
• Assumes responsibility for mentoring, coaching and staff development, acts as change champion; Contributes to performance evaluations.
• Champions department improvement activities.
• Leads, supports and mentors one of the three clinical advancement pathways (Clinical Quality Education, Leadership) where they serve as a mentor/preceptor/educator, lead a clinical quality or nursing research initiative to improve outcomes, or serve as a charge nurse or council chair and other related activities.
• Participates in the completion of required skill and competencies
• Assists with strategic planning; participates in activities related to recruitment and retention
• Performs skin tests, immunizations, phlebotomy and the initiation of peripheral venous access
• Evaluates and works to improve the process of patient care delivery within the unit and service centers
• Provides advanced assessment skills, integrating appropriate data sources to assess the signs, symptoms, reactions, behaviors, or general appearance to determine normal versus abnormal characteristics
• Provides expert comprehensive direct and indirect care to the specific populations needs, including cultural, spiritual, age, psychosocial, communication, gender, sexual orientation, economic, education, family and condition needs
• Identifies and assesses patient safety concerns with respect to age and developmental considerations. Intervenes to provide a safe environment and evaluates effectiveness of intervention for patient
• Performs all other duties as assigned.

Qualifications
Education

• Bachelor's Degree Bachelor's of Science Nursing (state accredited program) - minimum

Experience

• 5 years Experience within a specialty area - minimum
• 1 year Must be a Clinical Nurse III at CSMC for at least one (1) year, to be promoted within the clinical ladder - minimum

Licenses and Certifications

• RN State License - Upon Hire minimum California State RN License
• Basic Life Support (BLS) - Upon Hire minimum Require BLS unless a higher level of certification is required. As required by your specialty unit.
• Other – See Department Specifics - 1 Year minimum Other Competencies, as required by your specialty unit (ex. ACLS, PALS, fetal heart monitoring)
• ANCC Specialty Certification - 1 Year minimum Specialty Certification
