Registered Nurse – Jacksonville, IL

A healthcare organization is seeking a compassionate and dedicated Registered Nurse (RN) to provide skilled nursing care in a home-based setting. This opportunity is ideal for nurses who enjoy working independently while building meaningful relationships with patients and supporting their health and well-being.

Responsibilities:

• Conduct patient assessments and develop individualized care plans
• Provide direct nursing care and monitor patient progress
• Administer medications and coordinate treatment plans as appropriate
• Educate patients and caregivers on health conditions and care needs
• Maintain accurate and timely clinical documentation
• Collaborate with physicians and other healthcare professionals to support quality patient care
• Follow company policies, professional standards, and regulatory requirements
• Perform other duties as assigned

Qualifications:

• Active Registered Nurse (RN) license required
• Valid driver's license and reliable transportation required
• Current CPR certification preferred
• Previous nursing experience preferred
• Strong communication, organizational, and critical thinking skills
• Ability to work independently and manage patient needs effectively
• Compassionate and patient-focused approach to care
