Registered Nurse, Per Diem

Location: East Bay Area. Flexible territory assignments available, with patient visits coordinated around your preferred coverage area and proximity to home whenever possible.

Pay: $55.00–$75.00 per hour DOE

Position Summary

We are seeking an experienced Registered Nurse (RN) to provide skilled home health services, complete OASIS assessments, and coordinate patient care in collaboration with physicians, therapists, LVNs, HHAs, and caregivers. This role offers flexibility, autonomy, and the opportunity to make a meaningful impact in patients' homes.

What You'll Do

• Perform admissions, resumptions of care, recertifications, and discharge visits


• Complete OASIS assessments and maintain accurate clinical documentation


• Develop and update individualized care plans based on physician orders


• Assess patient status, monitor progress, and communicate changes to the care team


• Educate patients and caregivers on medications, disease management, and home care techniques


• Coordinate services with physicians and interdisciplinary team members


• Supervise and support LVNs and HHAs as needed


• Participate in case conferences and ongoing clinical education

Qualifications

• Active California Registered Nurse (RN) license


• Minimum 1 year of RN experience


• Home health and OASIS experience preferred


• Strong assessment, communication, and documentation skills


• Ability to work independently while collaborating with an interdisciplinary team
