Location: This is a full-time, on-site role based in Centennial, CO, ideally situated in the south Denver metro area with easy access from surrounding communities.
Note: This role is best suited for a currently licensed Registered Nurse (RN) with Long Term Care (LTC) or Skilled Nursing Facility (SNF) experience. Candidates without an active RN license will not be considered.

Job Description
Provide day-to-day clinical leadership and operational oversight for a skilled nursing/rehab unit, setting the tone for quality care and team accountability
Conduct daily rounds to monitor resident conditions, identify changes in status, and ensure timely intervention and communication across the care team
Complete pre-admission assessments and collaborate with the interdisciplinary team to support appropriate placement and individualized care planning
Lead and participate in care conferences, communicating clearly and compassionately with residents, families, and stakeholders
Audit clinical documentation, admission records, and nursing practices to ensure accuracy, compliance, and continuous improvement
Support and ensure adherence to PPS, MDS, OBRA, and state health department regulatory standards
Serve as a go-to clinical resource during emergencies and changes in resident condition, keeping the team calm and focused
Partner with the Director of Nursing to interview, onboard, and develop CNAs, LPNs, and RNs
Coach, mentor, and hold staff accountable in a way that fosters professional growth and team engagement
Evaluate and help implement tools and processes that improve care delivery and operational efficiency
Participate in on-call responsibilities as scheduled
Qualifications
Current Registered Nurse (RN) license in the state of Colorado (required)
2+ years of clinical experience in a Skilled Nursing Facility or Long Term Care setting (required)
1+ years of supervisory or management experience with direct report responsibilities
Working knowledge of PPS, MDS, OBRA, and Health Department regulations
Proficiency with Microsoft Office and healthcare-related software; EHR experience preferred
Strong interpersonal and communication skills across all levels, from frontline staff to families and external stakeholders
BSN preferred but not required
Ability to lift 50 lbs occasionally and 20 lbs frequently; must be comfortable with sitting, standing, and bending throughout the shift
