Job Description - Rehab RN/LPN- 6P to 6A

Clinical Outcomes:




  • Residents receive timely, individualized care from CNAs, with duties completed accurately and efficiently, contributing to high levels of resident satisfaction and wellbeing.




  • Resident care plans are implemented consistently and updated promptly in coordination with the Interdisciplinary Team (IDT), leading to improved clinical outcomes.




  • Medications (including IV, IM, and subcutaneous) are administered safely and in compliance with protocols, resulting in reduced medication errors and improved health outcomes.




  • Physician orders are accurately transcribed and promptly executed, ensuring continuity and safety of care.




  • Medication and treatment documentation is consistently completed, maintaining compliance and transparency in care delivery.



  • All nursing practices comply with federal, state, and facility regulations, resulting in high audit readiness and minimized legal or regulatory risk.

  • Electronic Medical Records (EMRs) accurately reflect the resident's current status, supporting data-driven decisions and compliance with documentation standards.

  • Medical records and resident information are properly transferred during transitions, maintaining continuity of care and compliance.


  • Residents and their families receive professional, compassionate support, improving trust and satisfaction with care.




  • Resident and family concerns are addressed promptly and escalated appropriately, leading to higher resolution rates and fewer formal complaints.




  • Team members complete required training and demonstrate competencies, contributing to a knowledgeable and compliant workforce.




  • Ongoing education, emergency preparedness participation, and infection control adherence results in a safe and resilient care environment.
