Our Client is hiring a fully remote Nurse Practitioner to support post-discharge and high-risk patients across Arizona and Georgia. This is a telehealth-based, on-demand role focused on transitions of care, readmission avoidance, and chronic condition management. The position offers genuine clinical autonomy within an interdisciplinary team model, with no commute and no facility requirement.

Who you are

You are a Nurse Practitioner with a background in primary care or internal medicine who is comfortable working with complex, chronically ill patients in a virtual environment. You have supported care transitions before, or you have managed high-risk patients in outpatient or community settings such as home visits or clinic-based care. You communicate clearly with field teams, coordinate across care partners, and document to regulatory standards without close supervision.

Who we are

Our Client is a healthcare organization providing virtual and field-based clinical services to patients who need support after a hospitalization or who carry significant chronic disease burden. The care model pairs remote NPs with field clinicians to deliver coordinated, timely care that reduces preventable readmissions and stabilizes patients in their homes. The team includes EMTs, Community Health Workers, and Social Workers, all operating under a shared clinical framework.

Your role

  • Conduct virtual clinical assessments for post-discharge and follow-up visits.
  • Complete medication reconciliation and update patient care plans.
  • Provide real-time clinical support to EMTs during in-home patient assessments.
  • Provide clinical oversight for tasks delegated to EMTs, CHWs, and Social Workers.
  • Address clinical quality gaps within NP scope of practice.
  • Coordinate follow-up appointments, referrals, and diagnostic testing.
  • Communicate with hospitals, primary care providers, and community care partners as needed.
  • Participate in interdisciplinary discussions and case reviews.
  • Document clinical findings in accordance with CMS, state, and accreditation requirements.

How you qualify

Required
  • Active Arizona Nurse Practitioner license.
  • Five or more years of clinical experience in primary care or internal medicine.
  • Outpatient or community-based experience — home visits or clinic settings.
  • Ability to support field clinicians remotely and in real time.
Preferred
  • AHCCCS registration.
  • Experience in telehealth or transitions-of-care settings.
  • Experience managing patients with multiple chronic conditions.
  • Familiarity with HCC coding or HEDIS measures.
  • Experience conducting annual comprehensive visits.

How you are supported

This is a fully remote position, structured around on-demand visits rather than a fixed daily schedule. You will work within a multi-disciplinary team that includes EMTs, CHWs, and Social Workers, with clear role definitions and clinical support structures in place. Our Client maintains compliance frameworks aligned with CMS and state regulatory standards, and the care model is designed so that NPs have the clinical information they need before each encounter.
