Residential Case Manager ASAM 3.5

The Case Manager provides comprehensive case management services to women participating in an ASAM 3.5 residential substance use disorder treatment program for pregnant and parenting women. This position plays a critical role in supporting participants throughout treatment by coordinating services, addressing barriers to recovery, facilitating access to community resources, confirming service linkages, and assisting with successful transitions to lower levels of care and community living.

• Complete comprehensive needs assessments and ongoing reassessments to identify participant strengths, needs, barriers, and service goals. 


• Develop, implement, and monitor individualized service plans in collaboration with participants and the treatment team. 


• Coordinate services with medical, behavioral health, prenatal, pediatric, educational, vocational, legal, and social service providers. 


• Track and monitor participant progress toward treatment, recovery, parenting, and self-sufficiency goals. 


• Assist participants in addressing barriers related to housing, transportation, childcare, employment, education, healthcare, insurance, and income supports. 

• Support participants in obtaining community resources, public benefits, identification documents, and other essential services. 


• Help participants navigate complex systems, including healthcare, child welfare, court, and social service agencies. 

• Coordinate services that promote family reunification, child safety, and family stability when appropriate. 

• Collaborate with child welfare agencies, family case managers, and other supports involved with participant families. 

• Begin discharge and transition planning at admission and update plans throughout treatment. 


• Coordinate transitions to lower levels of care, recovery housing, outpatient treatment, medical services, and community supports. 


• Support participants in developing comprehensive recovery and relapse prevention plans. 

• Maintain accurate, timely, and complete case management documentation in accordance with agency, state, federal, and grant requirements. 

• Document service coordination activities, referrals, linkages, barriers addressed, and participant outcomes. 


• Participate in multidisciplinary treatment team meetings and case reviews. 


• Maintain compliance with HIPAA, 42 CFR Part 2, and agency confidentiality policies. 


• Complies with federal, state, and local laws concerning duty to warn and mandated statutes including mandatory reporting of child abuse and neglect. 


• May facilitate groups addressing recovery-based rituals of daily living, including parenting skill development, employment, wellness, etc.  

• Participate in an on-call rotation. 

Minimum Requirements: 

• Bachelor's degree in social work, Human Services, Psychology, Counseling, Family Studies, or related field. 

• Minimum of two (2) years of experience providing case management, care coordination, or social services to individuals and families with complex needs. 

• Experience working with pregnant and parenting women, families involved with child welfare systems, or individuals with substance use disorders preferred. 


• Knowledge of community resources, public assistance programs, healthcare systems, and family service networks. 

• Valid driver's license and ability to meet agency transportation requirements.
