Job Description - Residential Supervisor - Senior

The Residential Supervisor – Senior is a key associate at Hope Network whose primary responsibilities include: providing overall operational leadership for the assigned residential environment(s) and staff in order to ensure excellent care through the provision of a cohesive team of trained staff who provide individualized care and rehabilitation supports, and who maintain a clean and organized homelike environment in a program that meets or exceeds all regulatory and funding requirements, as well as budgetary expectations. Serves as the Hope Network designated Licensee Representative/Administrator for multiple locations/+18 beds, or as the sole licensee representative for an operational region.


 


ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  1. Regular and predictable attendance is an essential requirement of this position. (This position is not available in a remote or hybrid model.)


 



  1. Staff management responsibilities include:

  2. Hire, train, supervise and provide timely performance feedback to the staff providing care within the program including Supervisors, Assistant Supervisors, direct care staff, assigned Rover Pool Staff and dietary or medical staff. Maintain staffing levels and training to support the needs of current census as well as the targeted admissions for the home. Ensure the provision of a comprehensive staff schedule in advance and adjust as necessary to meet the needs of the program. Assign staff roles and responsibilities within the program.


 



  1. Provide orientation to the program as well as just-in-time training to staff working in the home. Ensure staff complete the standard training for their positions within the orientation period. Proactively assign additional relevant training, as needed, including those provided by HN Training Department, Nursing, and clinical staff in order to develop specific skills to meet individual needs. This includes, but is not limited to, training on Behavioral/Substance Use plans, special diets, use of Care Forms, data collection, etc. Track staff skill and competency in the Hope Network tracking system.


 



  1. Communicate essential care and operational information to staff and solicit feedback/input from staff. Promote and encourage communication between clinical, supervisory and residential staff. Conduct routine staff meetings. Have representation at relevant resident meetings such as Patient Update, Annual Review meeting, etc. to ensure the highest quality of care and good communication in the program. Ensure the provision of all relevant information needed by direct care staff to provide excellent care in a timely manner to those we serve.


 



  1. Respond/follow up on the concerns/questions of the staff; promote positive morale, act as moderator in situations of staff dispute or disagreement involving the workplace. Provide and facilitate opportunities for staff growth. Consult with Human Resources representative as needed and appropriate.


 



  1. Participate in carrying on-call phone to be available after hours for staffing issues or emergencies on a rotating basis. This includes After-Hours Expectations.


 



  1. Ensure all Hope Network processes and systems to support excellent care are maintained in the program including, but not limited to:

  2. Medication administration: Ensure excellent health support through accurate medication administration and documentation. Ensure weekly medication inventory and reconciliation, and unused medications are disposed of properly and timely.


 



  1. Staff follow-through on resident care plans and support for personal care, supervision and protection.


 



  1. Documentation: Accurate and timely entry and uploads into the Medical Record including Residential Progress Notes, Bed Checks, Treatment Notes, patient tracking sheets, etc.


 



  1. Communication of patient medical appointments following the “Doctor Appointment Process” including communication back to the program and treatment team.


 



  1. Completion of required annual AFC paperwork, not less than annually including AFC Assessment Plan, Resident Care Agreement, AFC Health Care Appraisal, Standing OTC Order form, etc. (if applicable).


 



  1. Management of Resident funds, per policy guidelines.


 



  1. Assign and supervise the program’s Safety Officer activities, ensuring timely completion of all required inspections, drills and reports.


 



  1. Maintain high program census by actively working with the HN Neuro Rehabilitation admissions and marketing departments. Problem-solve the ability to admit hospital-based referrals within 24-48 hours, and all other referrals in a timely fashion to meet the goals of the referral source. In conjunction with admissions, nursing, and case management, ensure the staff and home have been properly prepared for each admission prior to date of admission, according to corporate procedures.


 



  1. Ensure new residents and families/guardians are oriented to the home. Communicate with residents’ families and guardians for coordination of approved visits, approved LOAs, or to provide critical updates during non-standard hours. Work with HN Case Manager to respond to complaints, concerns and requests related to the home in a timely and effective manner.


 



  1. Plan and ensure the implementation of quality leisure community-based activities for the residents utilizing creative and cost-effective ideas that target the varied interests of residents. Produce and circulate Activity Schedules. Incorporate ideas gleaned from Resident Council meetings. Work with HNNR Recreation Therapy as available to the program. Encourage family participation when possible.


 



  1. Serve as the Hope Network designated Licensee/Administrator for multiple licensed locations. Ensure compliance with Hope Network, AFC and CARF standards, rules and regulations. Communicate with AFC consultants, the Health Department and Bureau of Fire Safety inspectors as needed. Report incidents as required. Prepare and assist with AFC Licensing Inspections and AFC Special Investigations. Follow up after inspections/investigations with Correction Action reports within allotted time frames.


 



  1. Maintain a clean and homelike residential environment and vehicles through the routine cleaning activities of the program staff, replacement of furnishings as needed, and the coordination of professional repairs and maintenance by H.N. maintenance staff. This responsibility includes ensuring the cleanliness, maintenance and repair of facilities, grounds, furnishings, equipment and vehicles that are part of the assigned program.


 



  1. Manage revenues and expenses according to program goals. Manage and adjust direct care hours and staff shift assignments to meet resident census and current program needs.


 



  1. Other duties as assigned.


 


Addendum:



  1. Senior Residential Supervisor - Licensee Designee

  2. Senor Residential Supervisor II - Complex programs, sole person for a Region or overseeing programs consisting of 30+ beds or including one large AFC.


 


Position Qualifications:



  1. Required:

    1. Bachelor’s degree from an accredited university or institution of higher learning plus two years of experience in a human service agency; or

    2. 5+ years in residential-based supervisory role.




 



  1. Depending on the complexity of the environment, two years of experience working in a neuro rehabilitation program may be required.


 



  1. Work experience in a licensed setting required.


 



  1. Demonstrated ability to communicate in verbal and written formats to meet position requirements.


 



  1. Computer skills including use of Microsoft Office. Ability to learn and use electronic Medication Records, electronic Medical Records, payroll software, budgeting software and others as needed.


 



  1. Certification or ability to obtain Certification in First Aide/CPR.


 



  1. Certification or ability to obtain certification in Brain Injury fundamentals within 18 months.


 



  1. Valid Driver’s license with driving record acceptable according to Hope Network policy.


 



  1. Ability to work independently with flexible hours that may include working multiple shifts.
