What You'll Do

• Complete start-of-care, recertification, and discharge visits, including OASIS assessments


• Build individualized plans of care that help patients reach their goals


• Coordinate with physicians, therapists, and your LPN partner


• Teach patients and families the skills to thrive at home


• Document accurately and on time

What You'll Need

• Active Oklahoma RN license


• Valid driver's license and a clean driving record


• Home health experience preferred, but if you have strong clinical skills and a heart for patients, we'll teach you the rest


• OASIS experience is a plus
