Summary:

We are seeking an RN to provide personalized care and support to caregivers and their loved ones. No night shifts or weekends required, and no on-call duties! As an RN Care Manager, you'll assess needs, create individualized care plans, conduct home visits, and ensure exceptional consumer care.

What You Will Do:

• Conduct home visits in assigned territory to assess home safety, gather changes in health status, and provide clinical coaching
• Following an established care plan, provide ongoing coaching and support to caregivers
• Document and review records for accuracy and timeliness, ensuring compliance with regulatory and accreditation standards
• Perform other duties as assigned.

What You Will Bring:

• Must hold an active Registered Nurse (RN) License in MA
• Minimum two years of recent experience in direct care of elders or adults with disabilities 
• Ability to travel within assigned geographic area; valid driver’s license and automobile insurance
• Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a health care setting
• Additional state-specific qualifications may be required.
