RN Care Manager - Home Health

Our client is looking for a FT RN Case Managers for Home Health. Care Manager will work collaboratively with the home health team to provide client outreach, engagement, coordination of care, resource alignment and linkage to a defined case load. Holistic assessments, translating assessments to an individual care plan, and experience leading interdisciplinary team meetings are key skills for this role.

Requirements

Three (3) or more years of care management experience in a health care delivery setting required. 

-RN required

-Knowledgeable and skilled in evidenced based communication such as Motivational Interviewing, or similar empathy-based communication strategies.  Understanding of and sensitivity to multi-cultural community.  Deep understanding and knowledge of self-management philosophies and practices, especially as they relate to chronic medical conditions.  Awareness of the impact of unmitigated bias and judgement on health; commitment to addressing both.  Bilingual (English/Spanish) preferred.

Abilities Requirement

Able to sufficiently engage members and providers on the phone, including developing effective relationships that are phone-based.  Must be able to work as a member of a highly autonomous team, executing job duties and making skillful decisions as an independent team.

A valid California driver’s license, a reliable car, and valid automobile insurance are also required.
