RN Case Manager – Hospice
Pay: $75,000 – $90,000 per year
Job Type: Full-time | Field-based
Schedule: Monday–Friday, 8:30 AM – 5:00 PM

Responsibilities:
Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs
Develop, implement, and update individualized plan of care in partnership with patients and families
Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures
Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life
Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team
Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care
Qualifications:
Active Registered Nurse (RN) license in the state you work in
Proficiency with electronic medical records (EMR)
Strong organizational, communication, and critical-thinking skills
Ability to work independently while maintaining high standards of care
A compassionate, patient- and family-centered approach
Valid driver's license and reliable vehicle with current insurance
