Our Client is seeking a dedicated and compassionate Registered Nurse Case Manager to join their team in the Springfield, MA region. What You'll Do:

• Develop and Oversee Care Plans: Create and implement personalized treatment plans, provide skilled nursing care in patients' homes, and monitor progress.
• Partner with Families: Educate and coach parents about their child’s health, empowering them to play an active role in care.
• Leverage Digital Health Tools: Use technology to streamline communication, enhance efficiency, and track patient outcomes.
• Collaborate Across Teams: Work with interdisciplinary teams to ensure children achieve optimal health outcomes and support the organization’s mission.

About You:

• Qualifications:
  • Registered Nurse (RN) licensed in Massachusetts with 2+ years of nursing experience.
  • Preferably, 2 years of home health experience, with case management expertise being a plus.
  • Resides in the Springfield, MA area.
