Home Care Case Manager (HCCM) or Visit Nurse\n\n RN Admission Nurse (Central/Dayton)\n\n Documentation System Homecare Homebase\n\n \n \n \n\nGeneral Summary/Responsibilities: \nThe Home Care Case Manager (HCCM) or Visit Nurse is responsible for the provision and coordination of all components of home care services for patients. The HCCM visit nurse will provide care as ordered by the physician, follow policies, procedures, conditions of participation, and regularly report to their supervisor. The incumbent will facilitate the development and implementation of the plan of care as outlined by the physician. In addition, the incumbent will be responsible for all required documentation related to the care delivered and will communicate pertinent information regarding the patient to all appropriate health care team members.\n\n \n\nQualifications\n\nEducation\n\nMinimum Level of Education Required: Associate degree \n \nAdditional requirements: \n\uf0a7 Type of degree: Associates or Bachelors - Graduate of an accredited school of nursing \n\uf0a7 Area of study or major: Nursing \n\uf0a7 Preferred educational qualifications: N/A \n\uf0a7 Position specific testing requirement: N/A\n\nLicensure/Certification/Registration\n\n\uf0a7 Current Ohio Registered Nurse license in good standing. \n\uf0a7 CPR Certification\n\nExperience\n\nMinimum Level of Experience Required: 1 - 3 years of job-related experience \nPrior job title or occupational experience: N/A \nPrior specific functional responsibilities: N/A \nPreferred experience: One year clinical experience in acute care setting preferred/ HHC \nOther experience requirements: HCHB\n\nKnowledge/Skills\n\n\uf0a7 One year clinical experience in acute care setting preferred \n\uf0a7 Clinical experience must have occurred within the last three years unless employed currently in home care setting \n\uf0a7 If working as a specialist, documentation of the skills and certifications must be provided \n\uf0a7 Must be capable of standing, stooping, bending and lifting up to 40 lbs. \n\uf0a7 Must be self-directed and able to work with little supervision \n\uf0a7 Computer skills preferred \n\uf0a7 Home care experience preferred\n\n \n
