RN Psychiatric Case Manager - Dublin, OH

We are looking for RN Psychiatric Case Managers for a unique opportunity to provide one on one, psychiatric and skilled nursing care and treatment to patients in their homes.  As a licensed RN Psychiatric Case Manager, you will evaluate and access the medical, cognitive, emotional, psychosocial and psychiatric nursing needs of the patient to determine the appropriateness of care.  Our RN Psychiatric Case Managers coordinate care for the psychiatric patient by conducting regular review of treatment planning under the direction of a physician or psychiatrist, which includes nursing actions and interventions, goals and discharge summaries. We’ll provide you with an environment where you can work with other dedicated clinicians who are empathetic, engaged and compassionate.  

•    Current and unencumbered license to practice as a Registered Nurse specific to the state of Ohio. 
•    Minimum of one (1) year of clinical experience as a registered nurse.
•    A registered nurse must meet one of the following to meet the CMS requirements for psychiatric nursing in the home care setting: 
o    Master’s degree in psychiatric or mental health nursing.
o    Bachelor’s degree in nursing with one-year recent nursing experience in an active treatment unit in a psychiatric or mental health hospital or outpatient clinic.
o    Current CPR certification
o    Current driver’s license. 
•    Prefer candidate to have previous home health or hospice experience.
