As a Registered Nurse (RN) Case Manager, you’ll oversee home health aide services, ensuring they meet high-quality standards.

Registered Nurse Case Manager Responsibilities:

• Reports directly to Director of Operations


• Work closely with Clinical Coordinator to ensure our clients and employees receive BrightStar standard level of care


• Will work with both skilled and nonskilled cases/assignments


• Can provide field training/orient new employees to cases


• Can/will open skilled cases/assignments and maintain


• Field visits as needed


• Communication with Clinical Coordinator with any observed positive or negative in the field


• Responsible for assessments/evaluations/reassessments/training/supervisor visits


• Will assist with LRV assessments


• Other (TBD)

Registered Nurse Case Manager Requirements:

• Current unrestricted RN license in state for which care is being provided


• Minimum one (1) year of case management experience preferred.


• Minimum one (1) year of clinical experience within a home health agency, primary care clinic or health facility, preferred.


• With Reliable transportation with auto insurance


• Clean background and no criminal record


• Understands regulatory compliance with state and federal regulations


• Adhere to HIPAA and maintain client confidentiality.

Work setting:

• In home services/ Office

Weekly Schedule:

• Day shift (between the hours of 8 am to 8 pm)


• Monday to Friday


• Weekends as needed
