• Meets the discipline-specific productivity standard.
• Demonstrates competency in the care of patients during field observation by the clinical supervisor.
• Completes and submits documentation following established guidelines, as measured by a designated evaluation tool.
• Physician orders are present and written correctly.
• Documents follow-up of identified problems.
• Reviews and documents clinical progress toward goals.

• 1 year home health nursing experience in the past 5 years. and
• Exception: Will consider an applicant with1 year acute care Med/Surg, Tele, ICUNursing experience in the last 5 yearsonly when adequate staffing allows. and
• Registered Nurse: CA, upon hire and
• Basic Life Support - CPR, within 30 Days and
• Driver's License: CA, upon hire and
• Auto Insurance, upon hire
