RN-MCC ( Medical Care Coordinator)

Job Description:

Key Responsibilities:

• Provide comprehensive nursing care to patients in accordance with established policies and procedures.
• Coordinate and manage patient treatment plans, ensuring effective communication among the care team.
• Assess patient needs and develop individualized care plans that promote optimal health outcomes.
• Monitor patient progress and respond to changes in condition promptly.
• Educate patients and their families on health management and preventive care strategies.
• Collaborate with physicians and other healthcare professionals to ensure continuity of care.
• Maintain accurate and timely documentation of patient information in compliance with regulatory requirements.

Qualifications:

• Current RN license in the state of Wisconsin.
• Minimum of 2 years of nursing experience in a medical-surgical setting preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced, team-oriented environment.

Eligibility Requirements:

• Candidates must reside at least 100 miles away from Beloit, WI, to be eligible for submission. This is a non-negotiable requirement due to client restrictions.
