Job Description - Senior Director, Care Management (RN)
Senior Director, Care Management (RN) | New Opening
Lead care management across a regional health system, from a 330-bed acute care hospital to two rural critical access hospitals.
This is a newly opened senior leadership role for a care management executive who wants real scope and real influence. You’ll own care management strategy and operations across a 330-bed acute care hospital and two critical access hospitals (about 25 beds each). You’ll lead a team of 45 direct reports, including 2 supervisors, and your decisions will shape length of stay, readmissions, patient flow, and total cost of care across the region.
What makes this role stand out
Scope: Regional leadership across urban acute care and rural critical access settings.
A seat at the table: You’ll partner directly with physician leaders, CNOs, and the Executive Director of Care Management.
Room to build: You’ll standardize care management practice, drive process improvement, and shape how EPIC and reporting tools support your teams.
Flexibility: A hybrid schedule with 3 days onsite and 2 days remote.
What you’ll do
Lead care management managers and supervisors across all three hospitals.
Drive length-of-stay, readmission, and throughput improvement strategies.
Standardize care management across the service area to reduce total cost of care.
Coordinate care transitions between inpatient, outpatient, and post-acute providers.
Oversee complex cases, outliers, and ethics issues.
Monitor regulatory compliance through ongoing audits, and report key metrics to leadership.
Build strong partnerships with physicians, nursing leaders, and community providers.
What we’re looking for
A BSN.
An acute care hospital background. Experience from LTAC or behavioral/mental health facilities alone will not qualify.
Leadership experience in care management, case management, or utilization management.
Strong working knowledge of Medicare and Medicaid regulations.
5+ years at the director or manager level in case management, plus 6+ years in clinical or operational leadership.
A master’s degree in Nursing, Business, Healthcare Administration, or a related field, preferred
An RN license upon hire, and ACM or CCM certification within 1 year of hire.
Where adventure is part of everyday life
Picture a vibrant city of about 300,000 people, with a mountain range rising right at the edge of town and ocean inlets along the coast. You get big-city amenities: an international airport, a growing dining and arts scene, and pro-level healthcare. You also have world-class wilderness just minutes from your front door.
Outdoor access: More than 100 miles of paved trails run through town for biking, running, and cross-country skiing. Salmon fishing, hiking, glacier views, and a ski resort are all within easy reach.
The seasons: Summers bring nearly 20 hours of daylight, so you can hike after work until 10 p.m. Winters bring snow sports and the northern lights.
Your paycheck goes further: There’s no state income tax and no local sales tax.
