Social Services Assistant

Position Overview: Under the supervision of the Social Services Director, you will be responsible for addressing resident's psychological needs, assist with admissions and discharge planning, and meeting the needs of the residents. 

Key Responsibilities: 

•  Support residents and families through the admission, adjustment, and discharge processes.
• Complete social histories, assessments, and required documentation as assigned
• Maintain resident confidentiality and comply with HIPAA and all applicable federal and state regulations.

 Social Services Assistant Qualifications:

• Bachelor's degree or better in social work, human services, or a related field, preferred
• High school diploma or equivalent
• Skilled nursing experience, preferred
• Excellent communication and relationship building skills
