Social Services Director

Position Overview: As a Social Services Director, you will be responsible for addressing resident's psychological needs, assist with admissions and discharge planning, and meeting the needs of the residents.

Key Responsibilities:

• Support residents and families through the admission, adjustment, and discharge processes.
• Complete social histories, assessments, and required documentation as assigned
• Maintain resident confidentiality and comply with HIPAA and all applicable federal and state regulations.

Social Services Director Qualifications:

• Bachelor's degree or better in social work, human services, or a related field, preferred
• High school diploma or equivalent
• Skilled nursing experience, preferred
• Excellent communication and relationship building skills
