What you'll do:
Arranges adaptive equipment, clothing, personal items, and counseling services for residents
Maintains communication with families regarding health updates, care planning, and discharge planning
Coordinates referrals and access to outside services (transportation, voting assistance, talking books, etc.)
Assists residents with financial and legal matters, including Social Security, VA benefits, and preplanning resources
Promotes positive relationships between residents and staff while supporting resident dignity and individuality
Ensures confidentiality of all social services and resident information
Participates in interdisciplinary care plan conferences and contributes to care planning
Assists in resolving resident, family, or ombudsman grievances
Completes social service assessments, discharge planning, and MDS documentation in EMR
Documents resident social service needs, interventions, and measurable goals in care plans
Performs additional duties as assigned

What we're looking for:
Bachelors Degree in Social Work or Human Services
One year of supervised social work experience in a healthcare setting preferred
Experience with electronic medical records (EMR) required or preferred
Basic computer skills, including Microsoft Word and Excel
High school diploma or equivalent required
Master's level social work licensure preferred
CSW or LCSW helpful but not required
Exposure to body fluids and communicable diseases
Full range of motion required, including handling and lifting objects
Ability to lift, push, or pull 50-100 lbs. unassisted
Adequate vision and hearing (with or without assistive devices) to perform duties
