Social Worker

Winning Wheels is accepting applications for a Social Worker to join our one of a kind atmosphere! Winning Wheels is an 88 bed non-profit rehabilitation facility, specializing in young adults with traumatic brain injuries, and spinal cord injuries.

Responsibilities and Duties

Develop, implement, and review care plans designed to meet the psychosocial needs of the resident. Complete quarterly assessments, and necessary MDS portions as well.

Maintain ongoing communication with residents’ families, to ensure families are aware of changes, updates, etc., regarding their loved ones care.

Participate in discharge planning, and navigating resources/referrals appropriate and needed for the resident.

Perform administrative requirements, such as completing daily documentation, necessary forms, reports, etc., and submitting such to the Director as required.

This is a salaried position.

M-F with rotating Manager on Duty Weekends

Qualifications and Skills

Must have a bachelor's degree, or equivalent in social work or related field, or be a Licensed Social Worker.
