In collaboration with the Care Transitions team, provides patient and family advocacy and psychosocial interventions for patients referred to the care transition program. Strives to promote patient and family wellness, improved care outcomes, completion of advance care planning, and access to post-acute physician services, ordered hospital and/or out-patient services and appropriate community resources.Paid Parental Leave

ESSENTIAL FUNCTIONS:
Shall be responsible for providing care coordination including needs assessments and identification of care options, services, communication with patients and families in an interdisciplinary environment.
Gathers and manages patient data related to case management, care planning, and QI analysis.
Monitors delivery of care through patient rounds, documentation, identifying progress toward desired outcomes, maintaining ongoing communication with utilization review staff, etc.
Performs utilization review for all patients
Able to assist nursing staff with patient care when needed.
Assist in Rehab training and education to staff when needed.
Complies with hospital and legal requirements by fostering nursing practices that adhere to the philosophy, goals, and standards of care.
Abides by the Health Insurance Portability and Accountability privacy and security regulations regarding all aspects of Protected Health Information (PHI).

MINIMUM EDUCATION AND EXPERIENCE REQUIRED:
Completion of an approved school of nursing program.
One year experience as a registered nurse.
PREFERRED EDUCATION AND EXPERIENCE:
BSN
Care management experience.
LICENSURE OR REGISTRATION REQUIRED:
Current licensure as an RN in the State of Kansas.
PREFERRED LICENSURE OR REGISTRATION:
American Heart Association BLS
