Job Description:
Responsible for assisting patients and families with issues related to illness, recovery, and rehabilitation. Develops individualized psychosocial and discharge planning assessments to analyze social, emotional, and environmental factors affecting patient care. Provides medically related social services to support patients’ physical, mental, and psychosocial well-being. Guides staff in advocacy, protection, and promotion of patient rights. Performs psychosocial assessments and delivers individual, group, and family therapy in accordance with professional standards and regulations.
• Completes individualized psychosocial and discharge planning assessments to identify emotional, social, and environmental factors impacting patient care.
• Provides interventions based on assessment findings, including crisis intervention, counseling, education, advocacy, referral management, and mediation.
• Assists with screening, identification, and intervention in cases of suspected abuse, neglect, intimate partner violence, exploitation, and sexual assault, and coordinates with appropriate agencies.
• Collaborates with the interdisciplinary treatment team to develop, revise, and implement care and discharge plans, representing the social work perspective.
• Monitors, evaluates, and documents patient progress toward measurable goals, updating care plans as needed.
• Assesses financial needs and connects patients with community, state, and national resources to support care transitions.
• Maintains and utilizes resource guides for area agencies, community resources, and post-discharge services.
• Advocates for patient rights, assists with advance directives, and mediates conflicts or ethical dilemmas involving patients, families, or facilities.
• Participates in quality and performance improvement activities and maintains compliance with documentation and third-party payer requirements.
• Performs other duties as assigned.
Bachelor’s degree in social work, or a related field
Master’s degree in social work is preferred
0-2 years of experience in social work, or a relevant area
An equivalent combination of education and experience may be considered.
All experience must be paid and in the same related field.
Part-time and PRN experience will be prorated based on hours worked per week.
Volunteer work and internships for academic credit are not counted.
