Social Worker (LSW) - Per Diem - M-F - 8AM-4PM - Atlantic Visiting Nurse - Morristown, NJ

 Job responsibilities:

Primary Work Location - 465 South Street, Morristown, NJ 07960

Atlantic Health System is seeking a Per diem Social Worker, within the Atlantic Homecare & Hospice division.

Home visits with home care patients, providing disease counseling, end of life discussions with goals of care, community resource referrals, assistance with applications to community resources.

1. Performs comprehensive psychosocial assessments:

• Identifies issues that may have an impact on the safety of patients and families
• Identifies availability of support systems to meet patient/family goals.

2. Collaborates with other disciplines in the development of the plan of care:

• Provides input to the development of short-term and long-term goals
• Participates as an active member of the interdisciplinary team
• Collaborates with external resources in developing the plan of care.

3. Collaborates with other disciplines in the implementation of the plan of care:

• Counsel's patients and families in strategies to cope with disease process
• Participates with other disciplines in family conferences
• Provides referrals to appropriate community services.

4. Evaluates the effectiveness of social work services.
5. Maintains high standards of professional practice:

• Completes documentation according to agency guidelines
• Maintains current knowledge of community resources and eligibility requirements

6. Participates in the agency’s Performance Improvement program:

• Participates in utilization review activities.

7. Assists in the development of agency policies and procedures:

• Provides orientation to new employees
• Participates in developing/revising job functions.

8. Assists with growing agency business:

• Participates in community education and outreach
• Accepts other assignments as requested to support agency
• Other Tasks as assigned. 

Job Qualifications/Experience:

• Master's in Social Work from an accredited school of social work required
• Licensed in the state of NJ(LSW)
• State of NJ LCSW preferred
• 1-2 years’ experience in social work preferred 
• Expertise in community resources preferred
• Excellent customer service and communication skills.
• Use of personal vehicle is required for home visits. 
• Valid Driver’s License with a clean record required. 
• Bilingual a plus
