We’re looking for a Social Worker:

The Social Worker coordinates and performs the necessary administrative functions regarding the admissions/discharges of residents in Care Services (healthcare) and assists in the organization and planning of the Social Services provided for residents of our community. 

ESSENTIAL FUNCTIONS:

         Assists with the responsibilities and functions delineated in the discharge plan.

     Ensures that the administrative and clerical functions related to the admission and/or discharge of residents is carried out to completion by coordinating efforts with the Healthcare Team Members.

         Maintains appropriate contact with hospitals, doctors and outside agencies to facilitate admissions.

         Coordinates all forms needed for admissions.

         Maintains state required resident roster.

         Coordinates financial updates for private pay residents.

         Completes appropriate section of the MDS.

         Participate and attend all in-service training and education programs as scheduled.

         Other duties as assigned.

EDUCATION AND EXPERIENCE REQUIREMENTS:

         Bachelor’s degree in Social Work or closely related human services field required

         1-2 years related experience in a Healthcare setting.

         Two years’ experience in a similar social work position required.

         Two years of long-term care or rehabilitation experience desired.

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:

         Ability to read, write, speak and understand English fluently.

         Ability to meet or exceed the company’s attendance and punctuality standards.

         Ability to use Electronic Records and miscellaneous software and office equipment.

         Ability to understand and follow directions as given.

         Ability to work with minimal supervision.
