The successful Healthcare Coordination Specialist will work with Resident case plans and goals and provides on-going assessment and support. Assists the residents to utilize community resources to meet individual needs. Coach residents to identify their skills and abilities to promote personal growth.  Link residents with health care and care coordination services. T

Pay: $20 - $22.46/hour

MINIMUM QUALIFICATIONS OF POSITION 

• High school diploma or equivalent.


• Two-years work experience in a position that involved coordination among frequent interaction with patience/clients, health care providers, care coordinators, health plans, and other service systems critical to health.

PREFERRED QUALIFICATIONS OF POSITION

• Bachelor’s Degree in Healthcare Management, Public Health, or related field.

KNOWLEDGE AND SKILLS 

• Demonstrated strong understanding of how to work effectively with people with chronic disease, mental illness, disability, substance abuse and other health problems.


• Demonstrated and detailed knowledge of the health care system, key service providers and health care delivery system developments related to the health needs of residents.


• Communicate effectively both verbally and written to a diverse group of residents, staff, and external contacts while maintaining compliance with privacy laws.
