You will carry a manageable caseload, work autonomously in the community, and build real relationships with the people you serve — older adults and adults with disabilities who depend on you to help them stay safely in their own homes. The role is based in Taylor, MI with in-person onboarding for the first six months, then transitions to hybrid once training and performance requirements are met.
What You'll Do
As a Supports Coordinator/RN, you'll be the clinical backbone of our MI Choice Medicaid Waiver program — conducting in-home assessments throughout Wayne County (excluding Detroit, Hamtramck, Highland Park, Harper Woods, and the five Grosse Pointes) and partnering with social work colleagues to build individualized, person-centered care plans that reflect each participant's real needs and goals.
Assessment & Care Planning
Complete in-home assessments to identify medical, functional, psychological, financial, and environmental needs. Review medications and identify potential contraindications. Understand disease progressions and develop comprehensive, individualized Person-Centered Service Plans in line with Medicaid guidelines.
Clinical Collaboration
Partner with physicians, LPNs, hospice, community mental health, and other outside providers to coordinate care that supports each participant's independence. Use motivational interviewing to engage participants and families in their own care.
Ongoing Coordination & Documentation
Conduct reassessments collaboratively as an RN/SW team, link participants to community services and supports, maintain timely documentation, and meet productivity standards in compliance with all program requirements.
Professional Engagement
Participate in clinical supervision, case consultations, and staff development. Represent TIC professionally in the community and maintain all licensure requirements.
