Job Description - Talent Engagement Specialist - Enhanced Care Management

We’re looking for a recruiter who believes great hiring is great matchmaking — and that the right people in the right roles can transform care for the communities we serve. In this role, you won’t just fill seats. You’ll build talent pipelines that directly support our Enhanced Care Management (ECM) program and the individuals delivering care in the field.
This is not a desk-only role. Expect a true 50/50 split between field and desk work. If your ideal week is limited to online sourcing and virtual outreach, this is not the right fit.
You will partner closely with the ECM Program team to understand real-time hiring needs, workforce gaps, and community dynamics, ensuring we attract candidates who align with both the role and the populations we serve.

About Pacific Health Group
At Pacific Health Group, we are at the forefront of revolutionizing healthcare. You will play a vital role in this mission. We are dedicated to improving health outcomes by addressing social determinants of health and coordinating comprehensive community-based services through programs like Enhanced Care Management (ECM). If you are passionate about making a difference and have the skills to operate in a dynamic, mission-driven environment, we invite you to join our team.

What This Role Looks Like (Day-to-Day Reality)

  • Spend approximately 50% of your time in the field and 50% managing pipelines and coordination
  • Travel across assigned regions to support ECM hiring needs and outreach efforts
  • Partner directly with ECM leadership and field teams to align recruiting strategies with operational needs
  • Build and maintain active candidate pipelines for ECM roles
  • Drive candidate engagement, assessments, and follow-up throughout the hiring process
  • Manage multiple open roles simultaneously with speed, organization, and accuracy
  • Utilize recruiting platforms and tools to track candidate progress in real-time
This role requires strong time management, independence, and comfort working in fast-paced, people-facing environments

What You’ll Do

Community Outreach & Talent Engagement

  • Represent Pacific Health Group at job fairs, hiring events, and community-based outreach initiatives
  • Partner with ECM teams to identify key outreach locations and target populations
  • Build relationships with workforce boards, community organizations, schools, and training programs aligned with ECM services
  • Engage directly with candidates in the field to generate interest and build trust
  • Develop grassroots recruiting strategies within the communities we serve

Pipeline Development & Sourcing

  • Build and maintain strong pipelines for ECM roles through in-person outreach and digital sourcing
  • Source candidates through job boards, referrals, social platforms, and community networks
  • Develop partnerships with colleges, training programs, and workforce development organizations

Assessment-First Recruiting Approach

  • Introduce candidates to PHG’s assessment-first hiring model
  • Ensure candidates complete required assessments within 48 hours
  • Review resumes alongside assessment results and prepare Value Proposition summaries highlighting key qualifications, assessment performance, role alignment, and overall recommendation
  • Partner with ECM leadership to ensure candidate alignment with field expectations and program needs

Candidate Experience & Coordination

  • Serve as the primary point of contact for candidates throughout the hiring process
  • Coordinate interviews and ensure a seamless experience from initial outreach through scheduling
  • Maintain consistent communication, ensuring candidates feel informed and respected

Pipeline & Systems Management

  • Maintain accurate, real-time updates in Workable (ATS) and internal tracking systems
  • Track candidate progress, outreach activity, and pipeline health metrics
  • Ensure all recruiting activities align with compliance and fair hiring practices

How Success Is Measured

  • Quality of candidates advanced to ECM hiring managers
  • Candidate assessment completion rates and turnaround time
  • Pipeline health and role coverage for ECM positions
  • Speed and consistency of candidate follow-up
  • Effectiveness of community outreach and partnership development
  • Alignment of hires with field-based ECM role expectations

Who Thrives in This Role

  • Is energized by being in the field and engaging with people in person
  • Is comfortable working closely with operational teams and adapting to real-time hiring needs
  • Can manage multiple priorities and pipelines without losing momentum
  • Values quality over quantity in recruiting decisions
  • Is proactive, resourceful, and highly organized
  • Is passionate about connecting people to mission-driven, community-based work
