Job Description - Team Care Clinical Care Coordinator

Position Overview:


The Care Coordinator is responsible for supporting clients by assessing barriers and recommending resources and solutions relevant to each client. This position will work with clients and their families and in partnership with mental health providers, primary care providers, educators and other community providers to help clients achieve their health care goals.


Responsibilities: 



  • Work collaboratively with Fraser clinicians and clients and appropriate team members to support implementation of integrated treatment plans and assessments.

  • Assist individuals in obtaining needed services in the domains of mental health, social, educational, health, recreational, and related areas to improve child’s functioning.

  • Proactively assist individuals to identify barriers regarding access to recommended services.

  • Communicate effectively with the members of the individual’s care team to support continuity of care.

  • Conduct client appointments as needed during scheduled hours.

  • Provide consultation and communication with internal and external providers regarding shared clients.

  • Document, as appropriate, client related data, Social Determinants of Health data and referral information in Fraser’s Electronic Health Record.

  • Collaborate with internal and external providers to provide consultation and access for families to receive support.

  • Communicate in a respectful and professional manner with others involved with cases, including parents, educational staff, county social workers, probation officers, foster parents. 


Requirements:



  • Must be qualified in at least one of the following ways:


    • High School diploma or equivalent required with one year of relevant work experience – health care, mental health, customer service, administrative experience.



    • Bachelor’s degree or higher in one of the behavioral sciences or related fields from an accredited college or university preferred.

    • Certification as a Community Health Worker or Certification as a Family Peer Specialist preferred.

    • Possession of a valid Minnesota driver’s license and maintenance of a satisfactory driving record consistent with Fraser policy, required by specific needs of the individuals receiving services.

      Position Details:


      Location: Fraser Bloomington/Virtual Office & Community-Based


      Schedule: Monday-Friday, between the hours of 8 AM -  5 PM (full-time) 


      Pay: The starting pay range for this role is $22.60 - $23.00 an hour depending on qualifications.
