Job Description - Telemedicine Nurse Navigator - Full Time - Days
Position Summary:
The Telemedicine Nurse Navigator serves as the primary clinical liaison for patients, providers, and telemedicine specialists, ensuring seamless coordination of specialty telemedicine services across inpatient and outpatient settings. This position facilitates timely access to specialty consultations, coordinates patient care, provides education, manages referrals, and promotes continuity of care while supporting quality outcomes and patient satisfaction.
The Nurse Navigator works collaboratively with physicians, advanced practice providers, nursing staff, case management, ancillary departments, and external telemedicine specialists to improve access to specialty care for rural populations.
Basic Qualifications:
Education:
Requires an Associate’s degree in Nursing from an accredited school of Nursing or a Licensed Practical Nuse from an accredited vocational or community college.
Experience: Requires one year of work-related experience or an equivalent combination of education, training and experience.
Licensure, Registrations & Certifications:
Requires a current state license as a Registered Nurse or Licensed Practical Nurse and is listed in good standing with the state’s Department of Professional Credentialing.
Requires current certifications in Basic Life Support (BLS). (If BLS verification is not current upon hire, it must be obtained within three (3) months of hire.)
Essential Job Responsibilities:
Patient Navigation
Coordinate specialty telemedicine referrals from inpatient units, emergency department, and outpatient clinics.
Assess patient eligibility for telemedicine services.
Educate patients and families regarding telemedicine visits, expectations, privacy, and consent.
Ensure required documentation and informed consent are completed.
Prepare patients for telemedicine consultations.
Coordinate follow-up appointments, testing, and referrals.
Monitor completion of treatment recommendations.
Clinical Coordination
Collaborate with physicians to identify appropriate telemedicine consults.
Coordinate scheduling with specialty providers.
Facilitate communication between bedside clinicians and telemedicine physicians.
Ensure specialty recommendations are communicated to the treating provider.
Assist with implementation of treatment plans.
Care Coordination
Coordinate laboratory testing, imaging, and diagnostic studies.
Assist with transitions of care.
Reduce delays in specialty consultation.
Coordinate discharge follow-up.
Assist with reducing avoidable transfers.
Quality Improvement
Monitor telemedicine quality indicators.
Track consultation turnaround times.
Participate in performance improvement initiatives.
Assist with data collection and outcome reporting.
Identify workflow improvement opportunities.
Patient Education
Explain specialty treatment plans.
Provide disease-specific education.
Educate patients on medications and follow-up care.
Promote patient engagement and adherence.
Documentation
Document navigation activities in the electronic medical record.
