Transplant Coordinator Lead : Abdominal Transplant Administration

The Transplant Coordinator Lead maintains and supports nursing standards of care for a specific transplant population. Serves as a liaison, subject matter expert, and educational partner to support consistent standards of care across the transplant coordinator team. Collaborate with staff, the patient, and his/her family concerning patient evaluation and discharge planning. Maintains appropriate documentation for transplant candidacy and post-transplant follow-up care. Coordinates communications with the outpatient transplant population such as transplant clinic visits, community referrals, telephone consultations, outpatient treatments, and subsequent hospital admissions. Develops and coordinates a system to organize the flow of patients from initial referrals, through evaluation and education, to post-transplant follow-up and referral back to the community. Participates in collaborative, interdisciplinary research activities related to transplant patients or process improvement activities.

• Independently coordinates outpatient transplant patient care, and provides consultation on clinical and operational issues, including participating in clinic visits and analysis of diagnostic tests, monitoring patient and family’s compliance with medical regimen, reviewing laboratory and other pertinent data and communicates findings with attending provider.
• Serves as a lead resource to other transplant coordinators for complex outpatient coordination workflows and case management questions. 
• Coordinates donor procurement and recipient preparation by maintaining the UNOS waiting list, communicating donor and recipient status and coordinating admission process and preoperative preparation.
• Leads the development, revision, and standardization of transplant program policies, procedures, and educational programs for patients, families, nursing staff, and medical staff.
• Independently facilitates the evaluation and listing process of the potential transplant patient within the regulatory requirements of CMS and OPTN/UNOS.
• Maintains communication with referring physicians and insurance case managers and provides necessary information and records. 
• Provides education regarding the transplant evaluation, listing and organ allocation process in order to facilitate patient care.
• Is responsible for database management, such as the UNOS waiting list, required TIEDI forms, and internal departmental database Collects and maintains data during the transplant process as requested.
• Attends department quality meetings and leads in department QAPI activities.
• Provides informal guidance, coaching, and peer support to transplant coordinators I, II, and III.
• Partners with Transplant Manager or Director on staffing coordination, scheduling, operational support and timecard approval in accordance with delegated authority. 
• Serves as a lead resource for coordinating workflows, process improvements, daily workflow execution, case coordination, prioritization, and process consistency across the transplant coordinator team. 
• Performs other duties as assigned.

Education and Experience

• Required:
  • Bachelor of Science Nursing (BSN)
  • 5 years of transplant coordinator experience, including demonstrated experience in a lead, senior, or resource role. 
  • 1 year as a lead, senior, or resource role required.
• Preferred: 
  • Master of Science in Nursing or other healthcare-related field.
  • Experience with workflow development, process improvement, or standardization of initiatives within a transplant program preferred. Direct supervisory experience is not required.

Licenses Required

• Registered Nurse in the State of California - Current and in good standing.

Certifications Required

• BLS via American Heart Association.
• Required Certified Clinical Transplant Coordinator (CCTC) from the American Board of Transplant Coordinators (ABTC).
