Travel Social Worker

Rural hospital located several hundred miles northwest of Anchorage, AK seeking a full-time, Travel Social Worker to join their team.

Pay: $60.00/hour

Travel Social Worker Opportunity:

• Provide medical social work services to patients of all ages, including the patients located in the Acute Care Unit, Primary Care Clinic
  
  


• Respond to requests for medical social work services, including case management, discharge planning, nursing home/assisted living placements, risk man agreement, crisis intervention, and care coordination
  
  


• Social Worker Coordinate nursing home and assisted living placements for patients.
  
  


• 6-month contract
  
  

Travel Social Worker Qualifications:

• Must be a licensed Social Worker
  
  


• Performing medical social work, child protection or adult protection services or related field
  
  


• Ability to use and implement the psycho-social diagnostic process of assessment, diagnosis, planning, intervention and evaluation

• Rural, remote town northwest of Anchorage.
