Requirements
• Board -certified Psychiatric Mental Health Nurse Practitioner (PMHNP)
• Active, unrestricted New York State license
• Prior nursing home or long -term care psychiatric experience required
• Strong clinical judgment and ability to work independently
• Reliable transportation and willingness to travel statewide

Key Responsibilities
• Conduct comprehensive psychiatric evaluations and ongoing follow -up visits
• Diagnose and treat psychiatric and behavioral health conditions in geriatric populations
• Prescribe and manage psychotropic medications and monitor treatment outcomes
• Develop individualized treatment plans in collaboration with interdisciplinary teams
• Work closely with nursing staff, social workers, medical directors, and facility leadership
• Travel reliably to assigned locations throughout New York State
• Provide short -notice coverage when operational needs arise
• Maintain accurate, timely, and compliant clinical documentation
• Deliver patient -centered, trauma -informed, and evidence -based care
