Position Summary

Visiting Nurses Association (VNA) is seeking a clinically knowledgeable Weekend Intake Coordinator provide coverage on Fridays, Saturdays, Sundays and Mondays. This role serves as a key administrative liaison during weekend operations, ensuring timely intake, and appropriate service coordination.

The ideal candidate demonstrates strong clinical judgment, attention to detail, and the ability to assess referrals efficiently while maintaining strict confidentiality, dependable, professional and proper time management skills.

Key Responsibilities

• 
  

  Review and clinically assess Home Health and Hospice referrals for appropriateness and urgency

  
  


• 
  

  Obtain and verify referral documentation, physician orders, and insurance information

  
  


• 
  

  Collaborate with on-call nurses, case managers, and leadership to coordinate weekend admissions

  
  


• 
  

  Enter accurate referral and patient data into the electronic medical record (EMR)

  
  


• 
  

  Communicate professionally with referral sources, patients, families, and internal clinical staff

  
  


• 
  

  Ensure compliance with HIPAA, CMS, and state regulations

  
  


• 
  

  Maintain clear and timely documentation of all intake activities

  
  

Required Qualifications

• 
  

  Experience in Home Health, Hospice, or clinical care setting preferred

  
  


• 
  

  Strong understanding of clinical terminology and patient care workflows

  
  


• 
  

  Excellent communication and critical-thinking skills

  
  


• 
  

  Ability to work independently and manage time-sensitive referrals

  
  


• 
  

  Reliable weekend availability required
