About the Role
Washington State’s Wraparound with Intensive Services (WISe) program provides comprehensive, family-driven and youth-guided behavioral health services to youth with complex mental health needs and their families.
As a Care Coordinator, you’ll help youth and families navigate the WISe process by coordinating care, facilitating collaborative team meetings, and ensuring individualized care plans are developed and implemented. You’ll work closely with youth, families, and care teams to support meaningful outcomes and help youth thrive in their homes and communities. This role requires maintaining strict confidentiality when handling protected health information (PHI).

Work Environment
This position works across a variety of settings, including clients’ homes, the office, and while traveling between client locations. The office environment includes moderate noise, standard lighting, and occasional interruptions. Home visits may involve exposure to pets, residual smoke, or other household conditions, and smoking is prohibited during client visits. The role requires frequent travel for client visits and may also include travel for annual professional development.
Physical Demands
This position requires regular standing, sitting, speaking, hearing, and use of hands for computer and phone operations. The role requires sufficient visual acuity for computer and screen-based work. During home visits, the employee may need to sit on the floor, crouch, or kneel and may occasionally need to lift or assist children when necessary. Frequent sitting may also be required while traveling between client locations.
