Summary
The Youth Case Manager provides case management services to children, adolescents, and their families to support healthy development, stability, and success at home, school, and in the community. The Youth Case Manager works collaboratively with youth, caregivers, schools, and community partners to identify strengths, address barriers, connect families to resources, and support achievement of individualized goals.
This position requires strong organizational skills, effective communication, and the ability to build positive relationships with youth and families while working independently and as part of a multidisciplinary team. The Youth Case Manager works under the direct supervision of the Assistant Managers of Traditional Case Management and in collaboration with other Primary Health Solutions leaders.
A Day in the Life
Experience working with youth, families, schools, social services, behavioral health programs, community organizations, or related settings preferred. High school diploma or equivalent required. Associate degree, bachelor's degree, or relevant experience in human services, education, social services, behavioral health, childcare, youth development, or a related field is preferred but not required.
Candidates with lived experience, community-based experience, youth mentoring experience, or demonstrated ability to work effectively with children, adolescents, and families are encouraged to apply.
Valid driver's license, reliable transportation, and ability to travel within the community may be required.
