Youth Rapid Rehousing Program Mission

The Youth Rapid Rehousing (RRH) Program is a housing-first initiative with the goal of moving youth experiencing homelessness quickly into housing while also addressing their current needs through an integrated team composed of case managers, a peer support specialist, and a therapist. This team provides services to ensure the youth, ages 18-24, maintain their housing as well as help them work towards goals they create in relation to improving their mental and physical health, vocational or educational pursuits, building life skills, and connecting to the community to reach self-sufficiency.

Position Summary :

To provide consistent, individualized, and thorough case management services to clients who are moving out of homelessness and are living in housing throughout Salt Lake County. The case manager will support the clients in obtaining and maintaining stable housing and addressing current needs as defined by the client. The RRH case manager position is housed out of the VOA Youth Resource Center and includes mobile work meetings with clients out in the community/in their homes.

Essential Duties:

• Provide case management services to clients by assisting them in developing goals and a case management plan including but not limited to obtaining and maintain housing, and connecting to resources that support in improving their emotion, physical, mental, and community needs.
• Attend staffing/clinical staffing meetings weekly; participate and present client information at said meetings to achieve greater client success through collaboration as part of a multi-disciplinary team.
• Assist clients in locating, applying for, and securing housing after they have been triaged for the program through the coordinated entry process
• Assess (and regularly reassess) client needs utilizing appropriate, agency approved tools and identify priority needs to coordinate services as needed.
• Interact with all clients in a professional, trauma-informed manner while utilizing techniques such as motivational interviewing, Progressive Engagement, and appropriate boundary setting.
• Collaborate with Youth Housing Director in ensuring program fidelity, documentation accuracy, confidentiality, and intra-agency collaborations.
• Develop client files including case management plans, referrals, and/or discharge plans as appropriate utilizing a trauma-informed lens and with a client first focus.
• Utilize both internal tracking sheets and required case note web infrastructure in a timely and accurate manner.
• When appropriate, work with clients’ families to promote client self-sufficiency and continuity of care.
• Actively engage with clients by demonstrating cultural responsiveness and sensitivity.
• Participate in Youth Resource Centers on-call rotation
• Demonstrate a proactive willingness to learn and an openness to critical feedback.
• Utilize agency vehicles as needed to provide transportation to clients.
• Assist in moving clients in and out of housing.
• Perform other functions as necessary or assigned.
• Attend work on a punctual, regular, and predictable basis. Work must be performed on-site due to the need to provide direct client care.

A degree in the field of social services and two years’ experience working in the social services field or any combination of education, experience, and training which provides the following knowledge, skills and abilities:

• If driving is a part of the job, must be at least 21 years of age, possess a current driver’s license, have a good driving record, and be insurable under the agency liability policy.
• Must be able to pass Utah-DHS – Office of Licensing background screening and pre-employment drug screen.

Physical Demands

• Ability to lift and carry 50lbs.
• Ability to move around the facility, interacting with clients for extended periods.
• Work is generally performed in an office environment. May entail using a computer for extended periods of time.
