Chart Coordination Intern

The Chart Coordination Intern provides support to the Senior Chart Coordinator with chart reviews, documentation tracking, and chart completion activities. This role is designed to provide hands on experience with medical record documentation, electronic medical records (EMR) systems, and healthcare documentation processes. 

The intern will assist with routine chart related tasks while developing knowledge and skills that may support future advancement into a Chart Coordinator role. 

Essential Duties:

• Assist with reviewing medical records to identify missing or incomplete documentation.
• Support the Senior Chart Coordinator with tracking incomplete records (IR) and chart deficiencies.
• Assist with compiling daily IR information and organizing related emails and documentation.
• Assist with uploading designated reports and documentation to appropriate systems or vendors.
• Support the preparation of monthly charge router and/or notes reports as requested.
• Assist with communicating routine chart completion needs using approved communication templates and under the direction of the Senior Chart Coordinator.
• Maintain organized records of chart review activities, follow-up items, and completed tasks.
• Assist with tracking outstanding chart deficiencies and escalating unresolved items to the Senior Chart Coordinator.
• Support chart review activities within designated Electronic Medical Record (EMR) systems.
• Develop a basic understanding of documentation requirements and CMS guidelines as they relate to medical record completion.
• Participate in training and educational activities related to chart documentation, EMR workflows, and chart review processes.
• Provide backup administrative support to the Practice Support team for chart completion activities, as needed.
• Report questions, concerns, or potential documentation issues to the Senior Chart Coordinator for review.
• Perform other related duties as assigned.

 Education:

• High school Diploma or Equivalent

Experience:

• EHR/EMR (Electronic Health Record/Electronic Medical Record) experience preferred but not required. 
• Chart Auditing/Optimization experience preferred.
