What You’ll Do:
- Nutrition assessments and patient care planning.
- Educate patients regarding nutrition to promote wellness.
- Monitor weight, nutritional status, wounds, and nutrition support needs, including tube-feeding management when applicable.
- Collaborate with RDs, Physicians, Speech & Occupational Therapists.
- Help improve patient outcomes using “Food as Medicine”.
