Bereavement Coordinator, part-time

Longleaf Hospice and Palliative Care is locally-owned with offices located in Atlanta, Covington and Roswell, offering end-of-life care to those with life limiting illnesses in the communities we serve. We believe that this very special care is best provided by professionals who live and work in the community they serve. At Longleaf Hospice and Palliative Care, we integrate and address the diverse, seen and unseen needs of patients and their families. 

As a Bereavement Coordinator, you will be responsible for the management and coordination of all grief support programs and services provided by Longleaf Hospice through:

• Completing an initial assessment of patient and family to determine hospice care needs; providing a complete physical assessment and history of current and previous illness.
• Coordinating the assessment and delivery of grief counseling needs and services (one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers.
• Planning for Bereavement Program development, expansion and refinement through annual evaluation of services.
• Coordinating bereavement support to hospice personnel coping with work related grief, supervision of interns providing one-to-one counseling, and referral to community resources. Visits made for support as needed.
• Providing bereavement information and referral services to callers from the community. Expands and maintains community resource list as needed.
• Assisting in evaluating and planning services to meet hospice personnel support needs.
• Functioning independently but seeks and accepts guidance from other members of the Interdisciplinary Group, or from members of the community.
• Participating in hospice activities such as in-service education, hospice personnel meetings, and relevant committees.
• Advancing professional knowledge and skills Monitor, record and report changes in patients' condition to the IDG.
• Other duties as assigned.

• Bachelor’s degree or equivalent theological degree from a college/university; or a qualified Social Worker; or Spiritual Coordinator/Chaplain
• Current and continued CPR certification
• At least one or two years of recent home care experience; hospice experience preferred
• Effective verbal and written communication and problem-solving skills
• Responsible valid driver’s license. Reliable, insured, automobile for making home visits required
