Clinical Supervisor RN Home Health (Part-Time)

Empath Home Health, a member of Empath Health is seeking a resourceful and compassionate part-time Clinical Supervisor (Home Health) to work on our Home Health team onsite in Palm Harbor, FL.

Location: Palm Harbor, FL (fully onsite)

Schedule: Part-time role (20 hours per week). Two full day days (8 hours) and one half day (4 hours) during the week.

What you'll Do

• Provide oversight of all patient care services and personnel including: Making or assisting the scheduler with patient and personnel assignments, coordinating patient care, coordinating referrals, assuring that patient needs are continually assessed and assuring the development, implementation and updates of the individualized plan of care are including all physicians involved in the plan of care, and integrating the orders.


• Perform initial and ongoing comprehensive assessments of the patient’s needs: Outcome and Assessment Information Set (OASIS) assessments at appropriate time points.


• Supervise the nursing clinical staff engaged in direct delivery of home care services in the initial evaluation of patient’s needs, the written goal-oriented plan of care and the implementation of the physician’s plan of treatment.


• Coordinate the total plan of care with appropriate resources, conducting periodic reassessments of patient needs and revising the plan of care as needed.


• Ensure the care provided is based on a continual assessment of the patient that addresses physical, psychosocial, environmental and safety factors, physician’s orders, relevant medical history, functional status, and availability of caregiver.

What You'll Need

• Degree in Nursing required. Current license as a RN Registered Nurse in the respective state: ARNP or other advance practice nurse


• One (1) year home health care supervisory experience preferred with knowledge of state and federal regulations relative to home health.


• Prefers experience using Homecare Homebase (HCHB) EMR


• Strong assessment, critical thinking, and patient education skills
