Day Center Aide

Job Title: Day Center Aide
Department: Day Center                    
Location: ElderOne Silver Hill Day Center - 1000 Technology Pkwy, Newark, NY
Hours Per Week: 24hours
Schedule: Monday - Friday 8a - 4:30pm, no weekends                    
            

SUMMARY:
The Day Center Aide will assist the participant with ADLs as outlined in the care plan, in addition to providing assistance and supervision in a variety of activities at the Day Center.

RESPONSIBILITIES:

• Perform all activities listed on the Center Aide Care Plan, including personal care such as feeding, bathing, transfers/ambulation, meal set up, laundry, and prompting self-directed participants with medication administration
• Assist transportation with clothing and equipment for participant entrance and exit from the Center
• Review daily recreation calendar and assist Recreation Therapist with individual or group activities
• Report any concerns or changes in the participant’s physical condition, behavior, appearance or plan of care to the Clinic RN, Day Center Manager promptly
• Keep family and staff informed of participant activities and concerns
• Document accurately and completely each day at the end of the shift, all activities performed per the Center Aide Care Plan
• Complete annual skills competency program as per ElderONE requirements
• Perform other duties as required
   

REQUIRED QUALIFICATIONS:

• Must have the ability to complete initial training to demonstrate proficiency with ElderONE required skills competencies
   

PREFERRED QUALIFICATIONS:

• Valid NYS driver’s license preferred
• ElderONE Employment: 1 year experience working with the frail elderly population is preferred

PAY RANGE:

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
