Are you a compassionate Registered Nurse with a passion for behavioral health? Join our Home Health team and provide personalized, one-on-one care that helps patients safely remain in the comfort of their own homes.
As a Behavioral Health RN, you'll do more than treat medical conditions—you'll build trusted relationships, support emotional well-being, and empower patients facing challenges such as anxiety, depression, dementia, and other behavioral health needs. Every visit is an opportunity to improve quality of life and make a lasting impact.
What You'll Do
Conduct comprehensive nursing assessments and develop individualized care plans.
Provide skilled nursing care and case management in the home setting.
Monitor patient progress and perform ongoing reassessments.
Communicate patient status, assessment findings, and recommended interventions to physicians and the interdisciplinary care team.
Manage medical and behavioral health conditions through evidence-based nursing interventions.
Evaluate medication effectiveness, identify adverse reactions or contraindications, and educate patients on medication compliance.
Administer IM medications as ordered.
Respond appropriately to behavioral health or situational crises.
Complete accurate, timely clinical documentation, including assessments, progress notes, and discharge documentation.
Support patients and families with education, encouragement, and compassionate care throughout their treatment journey.
Requirements
What We're Looking For
Current, active Registered Nurse (RN) license in the State of California.
Behavioral health or psychiatric nursing experience that meets one of the following:
Diploma or Associate Degree in Nursing with at least 2 years of recent psychiatric/behavioral health experience, or
Bachelor's Degree in Nursing (BSN) with at least 1 year of recent psychiatric/behavioral health experience, or
Master's Degree in Psychiatric or Mental Health Nursing with at least 1 year of recent psychiatric/behavioral health experience.
Experience may be in home health, acute psychiatric care, or outpatient behavioral health settings.
Current CPR certification.
Valid driver's license, reliable transportation, and current auto insurance.
Ability to travel throughout the assigned service area.
For agencies operating under Medicare Home Health Benefit with Palmetto as Fiscal Intermediary, candidates must meet applicable Behavioral Health Home Care Nurse approval requirements.
