Position Summary
The Medical Social Worker (MSW) provides skilled social work services under physician direction and in collaboration with the interdisciplinary care team. This role is responsible for assessing patient needs, connecting patients and families with community resources, providing counseling and support, and helping patients overcome barriers to care.
The MSW serves as a patient advocate while ensuring documentation is completed accurately and in compliance with agency policies and Medicare/Federal regulations.
Key Responsibilities
Patient Assessment & Care Planning
- Conduct comprehensive psychosocial assessments to identify patient needs, strengths, and barriers to care.
- Participate in the development and implementation of individualized plans of care.
- Monitor patient status and communicate changes or concerns to the appropriate members of the care team.
- Assist in identifying solutions that support positive patient outcomes.
Resource Coordination & Advocacy
- Connect patients and families with community resources and support services.
- Assist with financial aid applications, benefit programs, and long-term planning needs.
- Advocate for patients to ensure their medical, emotional, social, and safety needs are addressed.
- Help patients access services that promote independence and quality of life.
Patient & Family Support
- Provide counseling and emotional support to patients and caregivers.
- Assist families with coping strategies related to illness, life transitions, loss, and grief.
- Educate patients and caregivers about available resources and support systems.
- Foster positive communication between patients, families, and healthcare providers.
Documentation & Compliance
- Complete accurate and timely clinical documentation in accordance with agency policies and Medicare/Federal guidelines.
- Maintain compliance with all applicable regulations, policies, and professional standards.
- Document assessments, interventions, patient progress, and care coordination activities.
Professional Responsibilities
- Attend mandatory meetings, trainings, and continuing education programs.
- Maintain effective communication with supervisors and scheduling staff regarding availability and schedule changes.
- Collaborate with interdisciplinary team members to support quality patient care.
- Perform additional duties as assigned.
Qualifications
- Current Medical Social Worker (MSW) license in the state of employment.
- Limited licenses are not acceptable.
- Master of Social Work (MSW) degree from an accredited program.
- Minimum one (1) year of experience as a Medical Social Worker in an acute care setting.
- Valid driver's license and current automobile insurance.
- Ability to travel throughout assigned service areas.
Preferred
- Previous home health or home care experience.
