Job Title: Part-Time Community Health Nurse - High Risk Perinatal Program

Southwest Human Development is seeking a compassionate and dedicated part-time Community Health Nurse to provide home-based developmental and health follow-up services to infants and young children who have experienced a Neonatal Intensive Care Unit (NICU) stay of five or more days. Serving families from birth through age three, the Community Health Nurse partners with parents and caregivers to promote healthy development, identify concerns early, and connect families with the resources and services they need to thrive.

This is a flexible, part-time opportunity compensated on a per-service basis. Employees are paid for each completed client service, such as appointments, visits, assessments, or other assigned services. Service volume and earnings may vary based on referrals, client participation, and scheduling availability.

In this role, you will:

• Conduct in-home and virtual visits with families to monitor child health, development, and family well-being.


• Build supportive relationships with parents and caregivers while providing education, guidance, and encouragement.


• Complete developmental and family wellness screenings using standardized assessment tools.


• Identify developmental, medical, or social concerns and coordinate referrals to early intervention and community resources.


• Support positive parent-child interactions and help families achieve health and developmental goals.


• Collaborate with healthcare providers, school districts, early intervention programs, and community agencies to coordinate services.


• Maintain an active caseload and provide ongoing follow-up from enrollment through program discharge.


• Schedule visits, communicate with families, and provide reminders to support engagement in services.


• Complete accurate and timely documentation, assessments, case notes, and reports in the electronic medical record system.


• Contribute to continuous quality improvement initiatives and ensure compliance with program, agency, and funding requirements.


• Maintain confidentiality and uphold HIPAA, safety, and infection control standards during all client interactions.


• Support program operations by maintaining equipment, following public health guidance, and adhering to productivity expectations.


• Engage in team meetings and other collaborative activities as required.
